You can send invitations and collect donations online.


  1. Use this donation form
    • Send to your guests or have on hand the big day.
    • Each guest who wishes to donate should fill out a form or donate online.
    • Checks should be made out to the Alzheimer's Association.
    • Collect all the forms and checks.


  2. Fill out the host report form
    After your event, fill out the host report form.

  3. Mail in donations within 30 days
    • Use one envelope.
    • Include (1) the host report form, (2) all donation forms and (3) all checks.
    • Send in one envelope to:

      Chicago, IL 60690

The Alzheimer's Association will send each donor an acknowledgment of their gift.



Go purple!

Use splashes of purple, the Alzheimer's Association official color, for invitations, party decor, food and drinks, signs, e-mail invitations, anything you can think of.

Fun food ideas:

  • Add blueberry, grape, blackberry or other purple-colored juice to your drinks.
  • Try a few drops of red and blue food coloring in dishes or desserts.
